BACTERIAL VAGINOSIS - A DIAGNOSTIC AND MORPHOMETRIC ANALYSIS

Abstract:

Aims and objectives of our study are to determine the reliability of the Pap smear in making the diagnosis of bacterial vaginosis, to determine the diagnostic characteristics features on Pap smear with that of clinical criteria in diagnosing bacterial vaginosis by using Gram stain as the diagnostic standard, and to analyse and compare the morphometric mean nuclear diameter in bacterial vaginosis and inflammatory smears by using normal smears as the standard. Materials and methods : A prospective study of 976 women attending the gynaecological out patients department from May 2009 to October 2009 were included in our study. All cases were screened using a pap smear, vaginal smear for gram staining and evaluated for clinical composite criteria. Smears for pap screening, Gram staining were reviewed by a pathologist and a microbiologist blinded to the microbiological and pap reports respectively. Nuclear morphometric analysis were performed on all cases of bacterial vaginosis and compared with the inflammatory and normal smears. Results: In a total of 976 cases, inflammatory smears formed the predominent population followed by bacterial vaginosis with white discharge per vagina and lower abdominal pain as the commonest clinical presentation. Morphometric analysis cofirmed maxium size of the mean nuclear diameter in bacterial vaginosis. Conclusion: Bacterial vaginosis is commonly seen in the second and third decade, pap smear remains as a better technique than clinical composite criteria. Nuclear morphometric studies highlights a significant increase in nuclear diameter in BV smears compared to the normal smears.
