COMPARISON OF RAPID SLIDE CULTURE AND FLUORESCENCE MICROSCOPY WITH CONVENTIONAL METHODS IN THE DIAGNOSIS OF PULMONARY TUBERCULOSIS.

Abstract:

Aims: Highly sensitive, rapid, simple and reliable diagnostic methods are needed for the early diagnosis of pulmonary tuberculosis(PTB), so as to initiate appropriate and timely treatment thereby preventing the spread of the disease and reducing its burden. This study was undertaken to compare three methods for rapid detection of Mycobacterium .tuberculosis in clinical material.

Material and methods: This was a prospective study, Which was carried out between June 2012 and January 2013. Single early morning and the second on the spot sputum samples were collected from 100 clinically suspected cases of PTB. The collected samples were concentrated and processed by Ziehl-Neelsen(ZN) staining, Fluorescence staining, rapid slide culture method (RSC) and culture on Lowenstein Jensen (LJ) media. Statistical Analysis: Sensitivity and Specificity. Results: Among the 100 early morning sputum samples, 26 samples (26%) were positive for ZN staining, 26 samples (26%) were positive for fluorescence microscopy, 36 samples (36%) were positive for culture on LJ media and 36 samples (36%) were positive for RSC method. The sensitivity and specificity of RSC method in comparison with growth on LJ media was 100%. Whereas the sensitivity and specificity of both fluorescence method and ZN staining method in comparison with growth on LJ media were 72 % and 100 % respectively. The results of the spot sample were same as early morning sample. Conclusion: The sensitivity and specificity of RSC method are equivalent to that of the gold standard LJ media. Moreover, it is more sensitive and equally specific compared to ZN staining and fluorescence staining method. This method is also easy to perform, cost effective and a time- saving rapid diagnostic method.
