Tuberculosis presenting as soft tissue swelling in thigh with bilateral inguinal lymphadenopathy is rare. A sixty year old Indian woman, coming from a rural background, presented with a subcutaneous swelling, measuring 5x4 cm in the mid-upper part of anterior aspect of thigh with a clinical diagnosis of Soft tissue sarcoma. FNAC from the thigh mass as well as lymphnodes showed caseating granulomas, which was confirmed as Tuberculosis by biopsy. Reliable FNAB diagnosis can prevent unnecessary, radical surgery.
